Abstract:

This article addresses some key aspects of the relation between textual fluidity and rewriting in the Second temple period on the basis of the comparison between the David traditions in Samuel and in Chronicles. While the discovery of Sam manuscripts in Cave 4 of Khirbet Qumran (especially 4QSama) has made likely that Chr used a source distinct from Sam MT, recent debates about the character of 4QSama and literary revisions in Sam MT and LXX suggest that the fluidity of the David traditions at the time of Chr’s composition may in fact have been even more significant than previously assumed. Building on this, the article compares the rewriting of these David traditions in Sam and Chr; in particular, it argues that although the scope of such rewriting is usually much more significant in Chr, there are several passages where Chr preserves an account of David which is less expansionist than Sam MT, and sometimes even than any other ancient version of Sam, as already observed by S. L. McKenzie and others. The implications of this finding for the characterization of Chr as rewritten composition are explored in the last part of the article. Chr’s relation to Sam emerges as a paradigmatic – albeit by no means unique – example of the complex dynamics that unite a rewritten composition to its source; this finding questions any simple, straightforward relation between the development of rewritten compositions and the textual stabilization of the source(s) on which these compositions are based. Furthermore, while it is possible to understand what features, precisely, would have led the ancient audience of Chr to see that work as a new composition, rather than as a variant edition of its source, it is dubious that such features may be used to define a literary genre proper.

Abstract:

The ascothoracid parasite Dendrogaster is reported from four boreal localities in the Western Hemisphere. Dendrogaster elegans Wagin infests the sea star Leptasterias polaris in Bristol Bay, Alaska, and in the Gulf of St. Lawrence; the eastern specimens have a shorter brood sac middle piece. A lectotype is selected and described for D. arctica Korschelt from L. groenlandica in the eastern Bering Sea; Siberian specimens previously assigned to this species differ from the type in many details. Immature specimens of an unidentified Dendrogaster species have been found in L. floccosa from the Davis Strait.

Abstract:

Abstract Background SAM is used as surgical prophylaxis during OLT due to its broad spectrum activity against Gram-positive,-negative and anaerobic pathogens. SAM resistance among Gram-negatives is rising, making dosage selection paramount to preventing surgical site infections. Current guidelines recommend a 3g dose, consisting of 2g ampicillin (AMP) and 1g sulbactam (SUL), every 2h. There are no data; however, describing SAM PK during OLT to support an optimized dosing regimen. Methods This was a single-center PK study of OLT patients receiving SAM for surgical prophylaxis at a dose selected by the anesthesiologist. Patients were excluded if they were undergoing simultaneous liver and kidney transplantation and had a CrCL < 30 mL/min at start of surgery. Up to 24 blood samples, along with times of pertinent events, were collected throughout the OLT. AMP and SUL plasma concentrations were determined. Population PK analyses were conducted in Pmetrics using R. Akaike information criterion (AIC) and visual inspection determined best model fit. Individual PK parameters were simulated to describe free AMP time above the MIC90 (fT >MIC90) of 32 mg/L. Results Five patients were enrolled. Participants had a mean ± SD age of 64 ± 7 years, body weight 82 ± 8 kg, CrCL of 75 ± 35 mL/min, and received various SAM doses (1.5-3g q2-3h). A 2 compartment model fitted the data better than a 1 compartment model for both AMP (AIC: 396 vs. 423) and SUL (AIC: 334 vs. 347). Final models included fractional clearance (CLf) terms on typical total body clearance (CLθ) to account for the placement of the portal vein clamp. AMP PK parameters (AIC: 372) were: CLθ, 9.7 ± 2.6 L/h; CLf, 0.73 ± 0.49; volume of central compartment (Vc), 7.2 ± 1.4 L; intercompartment constants (k12 and k21), 4.08 ± 3.28 and 2.63 ± 2.9 h-1, respectively. Final SUL PK parameters (AIC: 314) were: CLθ, 8.3 ± 2.5 L/h; CLf, 0.92 ± 0.55; Vc, 7.3 ± 1.6 L; k12, 4.60 ± 4.41 h-1, and k21, 4.07 ± 3.31 h-1. Exposures ranged from 58-96% with only 3g q2h providing nearly 100% fT >MIC90. Conclusion This is the first study to describe intra-operative SAM PK in OLT recipients and the effect of portal vein clamp on AMP and SUL clearance. These data will help guide optimized SAM dosing regimens for OLT surgery based on local MIC distributions for targeted pathogens. Disclosures David P. Delovanje človeka v boju, njegovo doživljanje vojne in nasilja so teme, ki niso pogoste v znanstveni literaturi. Med avtorji, ki so ta vidik predstavili še najbolj nazorno, so S. L. A. Marshall (1947), Richard Holmes (1985) in David Grossman (2004, 2009). Avtor je podobno raziskavo izvajal tudi sam. Izvajal je intervjuje z veterani preteklih vojn, pri čemer je ugotavljal, kako so jo doživljali in kakšne psihične posledice pušča delovanje v ekstremnih razmerah. V prispevku so predstavljeni rezultati študije in primerjava rezultatov avtorja s spoznanji omenjenih tujih avtorjev. Avtor ugotavlja, da so velike podobnosti med doživljanji angloameriških vojakov in vzhodnoevropskim ter balkanskih vzorcem, ki jih je zajel v raziskavo. To ga navaja na misel, da je odziv človeka na nevarnost univerzalen, vsaj v okviru zahodne kulture in sedanjega časa. A person's functioning in combat and how they experience war and violence are the topics not frequently addressed in scientific literature. S. L. A. Marshall (1947), Richard Holmes (1985) in David Grossman (2004, 2009) are the authors who have presented this perspective in the most illustrative way so far. The author has conducted a similar research himself. He has conducted interviews with the veterans of the past wars trying to establish how they experienced the war and what psychological consequences operations in extreme conditions leave on people. The article presents the results of this study and a comparison of the author’s results with the results obtained by the above-mentioned authors. The author notes great similarity between the experiences of the Anglo-American soldiers and Eastern-European and Balkan patterns he included in the research. This provokes the author’s idea of the universality of human response to danger, at least within the Western culture and the modern era.

Abstract:

In December 1995, leaf scald symptoms were observed in sugarcane (Saccharum sp.) cultivar B64277 in French Guyana. Symptomatic plants occurred both in a sugarcane germplasm collection near the road between Sinnamary and Saint-Elie and in a nursery near Sinnamary. Sugarcane imported from Martinique had been used to establish the germplasm collection that in turn had been used to establish the nursery. Ten-month-old mature plants in the germplasm collection had abnormal side shoots on the lower part of the stalks and suckers (nonmillable stalks) with white scalded areas on leaves. Leaves on 1-month-old shoots in the nursery exhibited chlorosis and white, pencil-line streaks. Samples prepared from symptomatic stalks from the two locations were plated on a selective medium (1), and two isolates of Xanthom*onas albilineans were recovered. Both of these isolates caused leaf scald symptoms on leaves of sugarcane cultivar B69566 inoculated by a decapitation technique, and belong to serovar 3 previously reported in the Caribbean from Guadeloupe, Martinique, and St. Kitts. The RFLP (restriction fragment length polymorphism) pattern of these two isolates was different from the 54 patterns among 218 other strains collected throughout the world (2), but similar to the pattern of a strain of serovar 3 from Martinique. This indicated that the pathogen might have been introduced with cuttings imported from Martinique. Three stalks of mature cane from varieties B5992, B64277, and R570 from the germplasm collection were tested for the presence of Clavibacter xyli subsp. xyli, causal agent of ratoon stunting disease. Immunofluorescence tests on sap (3) revealed the presence of the pathogen in the three stalks of B64277. All sugarcane plants in the nursery and the germplasm collection were destroyed by the use of glyphosate sprays in January 1996 in an attempt to arrest the spread of the two bacterial pathogens. In order to obtain healthy seed cane for future planting, a new germplasm collection of 0.6 ha and consisting of 11 cultivars was planted in January 1996 with disease-free, tissue-cultured plants provided by the CIRAD sugarcane breeding station in Guadeloupe. Abstract:

<span>El caracol rosa (<em>Strombus gigas</em>, Linnaeus, 1758) es una especie de importancia económica en el Mar Caribe, por lo cual, en la década de 1980 representó la segunda pesquería después de la langosta espinosa, razón por la que actualmente se encuentra en estado de sobrepesca. Con el objetivo de determinar la variación en la abundancia de larvas durante la época reproductiva, cuatro localidades del Caribe Mexicano “CM” (México: Puerto Morelos, Sian Ka’an, Mahahual; Belice: San Pedro) fueron muestreadas. Mensualmente, de mayo a octubre del 2008, se realizaron arrastres de plancton en cada localidad empleando una red cónica (300μm). Temperatura (°C), salinidad (ppm) y oxígeno disuelto (mg L-1) fueron registrados para cada sitio. Una densidad media larval de 0.34±0.87 larvas·10m-3 fue registrada entre localidades, con un pico de abundancia entre agosto y septiembre (0.82±1.00 y 0.76±1.68 larvas 10m-3, respectivamente). La densidad larval tuvo una correlación del 60% con la salinidad (r=0.6063, p&lt;0.05). El ANOVA de una vía mostró significancia estadística en tiempo (p&lt;0.05) y espacio (p&lt;0.05), donde Puerto Morelos tuvo los mayores registros durante el estudio (0.54±1.49 larvas 10m-3). Abstract:

Aim Osteoarthritis (OA) is caused by complex interactions between genetic and environmental factors. Epigenetic mechanisms control the expression of genes and are likely to regulate the OA transcriptome. We performed integrative genomic analyses to define methylation-gene expression relationships in osteoarthritic cartilage. Patients and Methods Genome-wide DNA methylation profiling of articular cartilage from five patients with OA of the knee and five healthy controls was conducted using the Illumina Infinium HumanMethylation450 BeadChip (Illumina, San Diego, California). Other independent genome-wide mRNA expression profiles of articular cartilage from three patients with OA and three healthy controls were obtained from the Gene Expression Omnibus (GEO) database. Integrative pathway enrichment analysis of DNA methylation and mRNA expression profiles was performed using integrated analysis of cross-platform microarray and pathway software. Gene ontology (GO) analysis was conducted using the Database for Annotation, Visualization and Integrated Discovery (DAVID). Results We identified 1265 differentially methylated genes, of which 145 are associated with significant changes in gene expression, such as DLX5, NCOR2 and AXIN2 (all p-values of both DNA methylation and mRNA expression < 0.05). Pathway enrichment analysis identified 26 OA-associated pathways, such as mitogen-activated protein kinase (MAPK) signalling pathway (p = 6.25 × 10-4), phosphatidylinositol (PI) signalling system (p = 4.38 × 10-3), hypoxia-inducible factor 1 (HIF-1) signalling pathway (p = 8.63 × 10-3 pantothenate and coenzyme A (CoA) biosynthesis (p = 0.017), ErbB signalling pathway (p = 0.024), inositol phosphate (IP) metabolism (p = 0.025), and calcium signalling pathway (p = 0.032). Conclusion We identified a group of genes and biological pathwayswhich were significantly different in both DNA methylation and mRNA expression profiles between patients with OA and controls. Abstract:

This article presents a theoretical review of the main evaluation approaches that marked the trajectory of educational evaluation in the international scenario. This is a bibliographical research that, based on the assessment literature, aimed at studying the various dimensions and different meanings that have historically constituted the educational evaluation field. Seven evaluation approaches are described: goal-based evaluation, evaluation based on scientific logic, value-based evaluation, decision-making evaluation, consumer-oriented evaluation, participant-centered evaluation, and qualitative evaluation. Each one of them is characterized according to its protagonists, objectives, conceptual approaches, theoretical orientations deriving from methodological assumptions, and the scholars who continued their respective approaches. Such international literature had a strong influence on Brazilian evaluation researchers who, based on these references, sought to support a field of knowledge that is still in the process of constitution and strengthening in our country.ResumoEste artigo apresenta uma revisão teórica das principais abordagens de avaliação que marcaram a trajetória da avaliação educacional no cenário internacional. Trata-se de uma pesquisa bibliográfica que, referenciada na literatura da avaliação, busca estudar as variadas dimensões e os diferentes sentidos que constituíram historicamente o campo da avaliação educacional. São descritas sete abordagens de avaliação: a avaliação baseada em objetivos, a avaliação baseada na lógica científica, a avaliação baseada no valor agregado, a avaliação a serviço da decisão, a avaliação orientada para consumidores, a avaliação centrada nos participantes e a avaliação qualitativa. Cada uma dessas abordagens é caracterizada segundo seus protagonistas, objetivos, enfoques conceituais, orientações teóricas decorrentes de pressupostos metodológicos e os estudiosos que deram continuidade nas suas respectivas abordagens. PrécisThe authors assume that costs can no longer be contained in the United States health care system and that the present system cannot be sustained beyond the near future. Three of the authors are affiliated with an applied economic research and consulting firm, and the fourth is president of the Healthcare Financial Management Association. They are trained in business and city planning. The bibliography lists articles from such journals as Hospitals, Business & Health, Business Insurance, and Medical Economics.The book is directed to members of hospital governing boards and other hospital administrators, but it will be of interest to students of health policy. Part I highlights tensions between what the authors call the worlds of doctors and hospital administrators struggling to survive, on the one hand, and health care planners worried about spiraling costs on the other. Part II contains five chapters that suggest reasons for growing costs and that criticize cost shifting as a remedy.In Part III the authors evaluate alternative health care systems by presenting four future scenarios: incremental change, universal access, consumer choice model, and single payer system. In these chapters they also approach the fundamental purpose of the book—to “help physicians, hospitals, and health plans take the next steps to position themselves for the future.” While not highly analytical, the book is a readable and thoughtful supplement to more abstract critiques of the impact of today's health care system on distributive justice.

Abstract:

In recent years, lily (Lilium spp.) has become an important ornamental crop in diverse regions of Mexico. Since 2005, unusual symptoms have been observed on lily plants grown from imported bulbs in both greenhouse and production plots at San Pablo Ixayo, Boyeros, and Tequexquinauac, Mexico State. Symptoms included a zigzag line pattern on leaves, dwarfism, enlargement of stems, shortened internodes, leaves without petioles growing directly from bulbs, air bulbils, death of young roots, atrophy of flower buttons, and flower abortion. Symptoms were experimentally reproduced on healthy lily plants by graft inoculation. Total DNA was extracted from 50 diseased, 10 symptomless, and 10 graft-inoculated plants by the method of Dellaporta et al. (2). DNA samples were analyzed for phytoplasma presence by two different nested PCR assays. One assay employed ribosomal RNA gene primer pair P1/P7 followed by R16F2n/R16R2 (1), whereas ribosomal protein (rp) gene primer pairs rpF1/rpR1 and rp(I)F1A/rp(I)R1A (4) were used in a second assay. A DNA fragment approximately 1.2 kb long was consistently amplified from all symptomatic plant samples only by both assays. A comparative analysis of 16S rDNA sequences (Genbank Accession Nos. EF421158–EF421160 and EU124518–EU124520) and rp gene sequences (EU277012–EU277014), derived from PCR products, revealed that phytoplasma infecting lily were most similar (99.9% to 16S rDNA and 99.7% to rp) to carrot phytoplasma sp. ca2006/5 and also were similar (99.8% to 16SrDNA and 99.2% to rp) to broccoli phytoplasma sp. br273. Both carrot and broccoli phytoplasmas were classified as members of aster yellow 16S rDNA restriction fragment length polymorphism subgroup 16SrI-B (3). Although infection of lilies by aster yellows (‘Ca. phytoplasma asteris’) subgroup 16SrI-B and 16SrI-C was reported from the Czech Republic and Poland, to our knowledge, this is the first report of ‘Ca. phytoplasma asteris’-related strains associated with lily plants in Mexico. Abstract:

In May of 2008, samples of southern highbush blueberry (interspecific Vaccinium corymbosum hybrids) exhibiting marginal leaf necrosis were collected at a farm in Interlachen, FL. The cv. Star showed severe leaf scorch, partial defoliation, and a generally unthrifty growth structure of many thin twigs as has been observed in Georgia (1). The block of Star plants was approximately 10 years old and incidence of the disease was 100%. The grower reported the planting had become increasingly unproductive in the most recent 4 to 5 years. Plants of the cv. Windsor also showed scorch symptoms and yellow-to-red discoloration of leaves. Proportionally fewer Windsor plants showed disease symptoms than Star plants and the disease was not as severe on this cultivar on the basis of visual estimates at the time. Each sample consisted of 5 to 10 cuttings of spring wood with attached leaves showing marginal necrosis taken from a single plant. Three samples, two from Star plants and one from a Windsor plant, were divided into two subsamples each. One subsample was submitted to Agdia for Xylella fastidiosa double-antibody sandwich (DAS)-ELISA assay (TSE XF; Agdia Inc., Elkhart IN). All three samples were reported as positive for X. fastidiosa by DAS-ELISA. A number of asymptomatic plants from this farm and other additional farms were tested in the same manner and results were negative. The other subsample was used for isolation of the causal bacterium. Petioles and main veins from symptomatic leaves were surface disinfested in 1% sodium hypochlorite, cut into segments (0.5 cm), and squeezed with forceps or pliers. Sap that exuded from the segment was blotted directly onto periwinkle wilt medium (2). Bacterial colonies consistent in morphology with X. fastidiosa that were DAS-ELISA positive were obtained from all three samples. One isolate from each sample was inoculated into four Star plants each with the pin-pricking method (3). Leaf scorch symptoms were first observed 8 weeks after inoculation. By 12 weeks after inoculation, all plants inoculated with the three isolates had developed symptoms, including defoliation. Plants inoculated without bacteria showed no symptoms. X. fastidiosa was reisolated from symptomatic plants. Bacterial leaf scorch is an important emerging disease that threatens the southern highbush blueberry industry in the south. On certain cultivars like Star, the potential to reduce yield appears to be great. Differences between cultivars are likely, but have not yet been explored. Abstract:

Virus-like symptoms were observed in several kudzu patches in Mississippi during a survey of viruses infecting soybean carried out in late summer/fall of 2013 as a part of a project funded by the Mississippi Soybean Promotion Board. Symptomatology consisted of chlorotic mottle and ringspots, vein-associated feathering, necrosis, and leaf deformation, which were often observed in combination on the same plant. In order to identify the virus(es) involved in the disease, young leaves from a symptomatic kudzu sample collected in Kemper County were crushed in 10 volumes of 0.1 M phosphate buffer (pH 7.2) and mechanically inoculated onto celite-dusted leaves of two soybean varieties (Asgrow AG4605 and AG4730), each represented by 10 plants. Sap from an asymptomatic kudzu sample from Oktibbeha County was used as a control. Both varieties reacted by systemic mottle, stunting, and apical leaf necrosis approximately 2 weeks after inoculation, while no symptoms could be observed in controls. Partially purified preparations from both symptomatic soybean cultivars exhibited the presence of putative intact and empty spherical virus particles ~30 nm in diameter. ELISA tests with antisera to several soybean viruses were performed on the original kudzu sample and inoculated AG4605 and AG4730 soybean plants. These tests revealed the presence of Tobacco ringspot virus (TRSV) in all symptomatic samples. In order to better understand the incidence of this virus in kudzu in Mississippi, a total of 127 samples from 28 counties were collected during October 2013 and tested using ELISA. A total of 11 samples collected in 8 different counties were positive for TRSV. To further confirm these results, one step RT-PCR test was performed on total nucleic extracts from all ELISA-positive and four negative kudzu samples using TRSV-specific primers (3). A specific PCR product of 766 bp was present in all ELISA-positive samples and positive controls, whereas no visible bands were present in negative samples. PCR products generated from samples, collected in Kemper, Tippah, and Jefferson Davis counties, were cloned and custom sequenced. Pair-wise comparisons indicated conserved nucleotide (95 to 98%) and amino acid (98 to 99%) contents among sequenced products. Kudzu isolates from Mississippi shared 91 to 96% and 98 to 99% conserved nucleotides and amino acids, with TRSV sequences currently available in the NCBI/GenBank database. This is the first report of TRSV infection of kudzu in Mississippi. The possible implications to the soybean industry are yet to be determined since kudzu occupies approximately 202,000 ha in Mississippi and TRSV has historically been reported associated with bud blight in soybean (1). Nonetheless, results of our study, along with the recent report from Louisiana (2), strongly suggest that kudzu, due to its widespread distribution in the region, may represent a major reservoir of TRSV in the southeastern United States. Abstract. Politically, School Based Management (SBM) becomes the mouthpiece of all issues in the field of education that will be portrayed in schools, because the school is the last network of educational bureaucracy. SBM is also a form of operationalization of the decentralization or education autonomy policy in relation to regional autonomy. Theoretically, SBM is also a concept that offers autonomy to schools in order to improve quality, efficiency and equity of education in order to accommodate the interests of local communities as well as establishing close cooperation between schools, communities and governments. Operationally SBM is an idea that places the authority of school management in a system entity. Based on the above view, this article outlines the basic framework of SBM as a strategy for improving the quality of education. With SBM, principals, teachers and learners get the opportunity to innovate and improvise in schools related to curriculum, learning, managerial and others. So the principal serves as an educator, manager, administrator, supervisor, leader, innovator, motivator, figure, and mediator. SBM also calls for the creation of new institutional arrangements and institutions, including: the establishment of school boards, development of school strategy planning, develop of annual school planning, internal monitoring and self-assessment, annual reporting, school opinion surveys of school stakeholders. MBS juga sebagai bentuk operasionalisasi dari kebijakan desentralisasi atau otonomi pendidikan dalam hubungannya dengan otonomi daerah. Secara teoretis, MBS merupakan suatu konsep yang menawarkan suatu otonomi kepada sekolah dalam rangka meningkatkan mutu, efisiensi dan pemerataan pendidikan agar dapat mengakomodir kepentingan masyarakat setempat serta menjalin kerja sama yang erat antar sekolah, masyarakat dan pemerintah. Secara operasional MBS merupakan gagasan yang menempatkan kewenangan pengelolaan sekolah dalam suatu keutuhan entitas sistem. Berdasarkan pandangan di atas, artikel ini menguraikan kerangka dasar MBS sebagai strategi dalam peningkatan mutu pendidikan. Dengan MBS, kepala sekolah, guru dan peserta didik mendapatkan peluang untuk melakukan inovasi dan improvisasi di sekolah berkaitan dengan masalah kurikulum, pembelajaran, manajerial dan lain-lain. Abstract:

Corn production in several areas of Brazil recently has been seriously afflicted by a disease commonly referred to as “red stunt,” characterized by stunting and leaf reddening. Early observations that a phytoplasma was associated with the disease were confirmed through molecular analysis, which revealed the presence of maize bushy stunt phytoplasma (MBS) (1). Another disease of corn, corn stunt, is considered to be caused by one or more of a complex of MBS, corn stunt spiroplasma (CSS), and Maize rayado fino virus (MRFV), which can all be transmitted simultaneously by their leafhopper vector Dalbulus maidis (Delong & Wolcott). The contributions of CSS and MRFV to the recently described “red stunt” disease in Brazil are unknown. A virus serologically related to MRFV, Brazilian corn streak virus, was first identified in Sao Paulo State, Brazil, in the early 1970s; serological studies indicated that isolates of Brazilian corn streak virus were related to, but distinguishable from, MRFV isolates from other Latin American countries (4). Therefore, there was a high probability that MRFV would be found in maize tissues collected from plants exhibiting symptoms of “red stunt” disease. Maize leaf samples exhibiting symptoms of “red stunt” disease were collected and preserved by drying from four Brazilian States during 1995 and 1996 (1). Total nucleic acid extracts were prepared from dried leaf tissue and aliquots of the extracts were spotted onto a nylon membrane, which subsequently was hybridized to an MRFV-specific cRNA probe (3). Of the 37 samples tested for the presence of MRFV by nucleic acid hybridization, 16 were positive for MRFV. It was present in some, but not all, samples that were positive for MBS (1). MBS was detected in six, and CSS was detected by polymerase chain reaction (PCR) (2) in 12 samples. In 8 of the 37 samples, both CSS and MRFV were present, 4 of 37 were positive for MBS and MRFV, and in 3 of 37 samples, all three pathogens were detected. Therefore, there is not a clear correlation between the presence of MRFV and the symptoms of “red stunt.” The coat protein gene and adjacent 3′ nontranslated region of MRFV were amplified from infected tissues by reverse transcription-polymerase chain reaction (RT-PCR) using MRFV-specific primers (3). Three cloned PCR products were sequenced (deposited at GenBank under accession nos. AF186177 to AF186179), which revealed that the nucleotide sequences of the Brazilian isolates were 98% sequence identical and shared 90 to 97% identity with other MRFV isolates (3). Phylogenetic analysis of the sequences revealed close relationships to MRFV isolates from Peru and Bolivia, which neighbor Brazil (3). The contribution of MRFV to the stunting and leaf reddening symptoms exhibited by maize plants with “red stunt” disease is unknown. Of the 37 samples examined in this study, MRFV was detected in 16. A more complete epidemiological study of the association of MBS, CSS, MRFV, and their insect vector with “red stunt” disease will provide information on the significance of these pathogens in the current disease outbreak. Abstract:

Background:Rituximab (RTX) is an effective therapy for induction of remission in ANCA-associated vasculitis (AAV). However, the effect of RTX is not sustained, and subsequent relapse rates are high, especially in patients with a history of relapse.Objectives:The RITAZAREM trial is an international, multi-center, open-labelled, randomized, controlled trial of patients with AAV with relapsing disease comparing the efficacy, after induction of remission with RTX, of two relapse-prevention strategies: repeat dosing of RTX or daily oral azathioprine (AZA).Methods:Patients with AAV were recruited at the time of relapse and received induction therapy with RTX and glucocorticoids. If remission was achieved by month 4, patients were randomized in a 1:1 ratio to receive either RTX (1000 mg every 4 months for 5 doses) or AZA (2 mg/kg/day) as maintenance therapy. Patients were followed for a minimum of 36 months, with the primary outcome being time to disease relapse.Results:190 patients were enrolled and 170 randomized at 4 months (85 to RTX; 85 to AZA). The data are complete on all patients up to at least month 24. After adjustment, none of the randomization stratification covariates (ANCA type, glucocorticoid induction regimen, or relapse severity) had a significant differential effect on the primary outcome. By 24 months after entry, 20 months after randomization, 11/85 (13%) patients in the RTX group had experienced a relapse compared to 32/85 (38%) patients in the AZA group. 19/85 (22%) patients in the RTX group and 31/85 (36%) patients in the AZA group experienced at least one severe adverse event (SAE). 25/85 (29%) and 42/85 (49%) patients in the RTX group developed hypogammaglobulinaemia (IgG <5g/l) and non-severe infections respectively, compared to 21/85 (25%) and 41/85 (48%) in the AZA group.Figure 1.Relapse-free survival in RITAZAREM trial: rituximab versus azathioprineConclusion:In the RITAZAREM trial, following induction of remission with RTX, RTX was superior to AZA for preventing disease relapse in patients with AAV with a prior history of relapse. Background:Remission, LDA and LDAS have been proposed as treatment goals for SLE. However, the independent impact of these states on damage accrual has not been fully evaluated.Objectives:To determine the independent impact of remission (both off & on treatment), LDA, and LLDAS on damage accrual.Methods:We studied a long-term longitudinal multinational SLE cohort, including patients completing at least two annual assessments. Remission off-treatment was defined as a SLEDAI (excluding serology) =0, without prednisone and immunosuppressive (IS) drugs. Remission on-treatment was defined as a SLEDAI (excluding serology) =0, prednisone daily dose<=5 mg/d and maintenance IS drugs. LDA was defined as a SLEDAI (excluding serology) <=2, without prednisone or IS drugs. LLDAS was defined as a SLEDAI <=4 with no activity in major organ systems, with no new features of lupus disease activity compared to the previous assessment, prednisone daily dose<=7.5 mg/d and maintenance IS drugs. Antimalarials were allowed in all groups. Damage accrual was ascertained with the SLICC/ACR damage index (SDI). Univariable and multivariable generalized estimated equation (GEE) negative binomial regression models were used. To create mutually exclusive groups, disease activity was divided into five states: remission off-treatment, remission on-treatment (minus remission off treatment), LDA (minus remission), LLDAS (minus remission and LDA) and not-optimally controlled. The proportion of the time that patients were in the specific state at each visit since cohort entry was determined. Possible effect modifiers and confounders adjusted for included sex, age at diagnosis, race/ethnicity, education, baseline disease duration, follow-up time, the highest-ever glucocorticoid dose prior to cohort entry, antimalarials and SDI. Time-dependent covariates were determined at the same annual visit as disease activity state; the outcome was the increase in the SDI and it was assessed at the subsequent visit.Results:There were 1,652 patients, 1464 (88.6%) were female, mean age at diagnosis was 34.6 (SD 13.4) years and mean baseline disease duration was 5.5 (SD 4.1) months. Patients had a mean follow-up of 6.5 (SD 4.3) years, 11686 visits were included. 763 patients (46.2%) had an increase in SDI score ≥1 during follow-up. 2483 (21.2%) of the visits were classified as remission off-treatment, 2276 (19.5%) as remission on-treatment, 544 (4.7%) as LDA, 657 (5.6%) as LLDAS and 5726 (49.0%) as not-optimally controlled. Being in remission off-treatment, remission on-treatment, LDA and LLDAS were predictive of a lower probability of damage accrual [remission off-treatment IRR=0.403, 95% CI 0.301-0.541); remission on-treatment IRR=0.313 (95% CI 0.218-0.451) LDA: IRR=0.469 (CI 95% CI 0.272-0.809); LLDAS IRR=0.440 (95% CI 0.241-0.803)]. RESUMENEl objetivo de este artículo es reflexionar sobre los mecanismos de financiación y de control de las instituciones religiosas por los laicos en las primeras décadas de la conquista y colonización de Hispanoamérica. Investigar sobre la inversión laica en lo sagrado supone en un primer lugar aclarar la historiografía sobre laicos, religión y dinero en las sociedades de Antiguo Régimen y su trasposición en América, planteando una mirada desde el punto de vista de las motivaciones múltiples de los actores seglares. A través del ejemplo de restituciones, donaciones y legados en losAndes, se explora el papel de los laicos españoles, y también de las poblaciones indígenas, en el establecimiento de la densa red de instituciones católicas que se construye entonces. La propuesta postula el protagonismo de actores laicos en la construcción de un espacio cristiano en los Andes peruanos en el siglo XVI y principios del XVII, donde la inversión económica permite contribuir a la transición de una sociedad de guerra y conquista a una sociedad corporativa pacificada.PALABRAS CLAVE: Hispanoamérica-Andes, religión, economía, encomienda, siglos XVI y XVII.ABSTRACTThis article aims to reflect on the mechanisms of financing and control of religious institutions by the laity in the first decades of the conquest and colonization of Spanish America. Investigating lay investment in the sacred sphere means first of all to clarifying historiography on laity, religion and money within Ancien Régime societies and their transposition to America, taking into account the multiple motivations of secular actors. The example of restitutions, donations and legacies inthe Andes enables us to explore the role of the Spanish laity and indigenous populations in the establishment of the dense network of Catholic institutions that was established during this period.
